Care co-ordination and Moving
Supporting with downsizing and moving, finding home care services or helping individuals and families with finding the right care home. If you would benefit from accompanied visits to see care homes, please get in touch.
Consulting and Advocating
Strong consultation and guidance with the next steps. Support with hospital admission and discharge. Guiding families through referrals to social care, applications for benefits (Attendance Allowance, Pension Credit), applications for Lasting Power of Attorney (LPA).
Vitality and Living a Flourishing Life
Providing help and advice in aspects of wellbeing including organising events and outings, meal planning and grocery support, bill management, budget planning, helping with staying connected to family members and organising hobbies.

Welcome to Flourish Eldercare. I’m Delyth and I specialise in supporting families with the complex challenges of caring for aging loved ones. With over 12 years of experience managing elderly care needs—navigating the NHS, social care systems, and supporting with benefits and lasting power of attorney applications —I offer clear, professional guidance and tailored solutions.
With a background in consulting on special needs and disabilities (https://delythsendconsulting.com/) and counselling, I understand the need to protect and empower the most vulnerable. I also understand the pressures of balancing caregiving responsibilities with work and distance from loved ones.
Flourish Eldercare offer a comprehensive range of services to support families in every aspect of caring for older people, including decluttering and home organisation to ensure a safe and functional living environment, providing guidance on essential matters such as applying for benefits (e.g., Attendance Allowance, Pension Credit), setting up Lasting Power of Attorney (LPA) applications, and coordinating home care services, to work with multidisciplinary teams ensuring a holistic approach to care.

Support packages
We offer a range of packages including:
Initial Consultation – £250
- Includes a home visit or video consultation to understand needs, priorities and risks, followed by a written summary with suggested next steps.
Essentials Package – £475
5 hours of support used flexibly over one month for practical support including:
- Attending appointments, wellbeing planning or home organisation.
Care Navigation and Co-ordination Package – £950
- 10 hours of support used flexibly over two months. Suitable for families needing help with care agencies, Adult Social Care referrals, professional liaison, appointments, moving planning or care home research.
Consulting and Advocacy Package – £1100
- 10 hours of support used flexibly over two months. Suitable for more complex family situations requiring advocacy, planning meetings, written summaries, liaison with professionals, support with social care processes, or advice around next steps. Will include support for elderly members who have been sectioned under the Mental Health Act.
Monthly Membership (Best for Ongoing Support)
Ideal for clients who need occasional assistance and a limited amount of support each month.
